what is it?
The PHN program offers group programs with a mental health and wellness focus.
These tend to be one-off session or short-term group programs which are offered at no or low cost to the community. The funding for these programs are provided by the Darling Downs & West Moreton Primary Health Network.
who?
This project provides limited low intensity counselling support plus small group workshops aimed at improving the mental health and wellbeing of any aged person resident in the Granite Belt region.
how does it work?
The types of programs offered through PHN are varied and change each year. Some offered workshops included arts (art therapy) programs, healthy lifestyle and nutrition programs, training for community members to support their own mental health or to support others.
Those who would like to participate are asked to simply book through reception at the Granite Belt Neighbourhood Centre either by phone or by email.
It is, however, a requirement of the funding body that all participants give their consent for deidentified information from the registration form be entered in the Darling Downs & West Moreton Primary Health Network database. The registration form is completed on arrival at the program or you can ask for a copy of the registration before the program date.
when?
Granite Belt Neighbourhood Centre is open weekdays from Monday to Friday from 9am to 4pm. The PHN worker is employed to co-ordinate these programs and works one day per week. The best way to contact the PHN worker is via email: phnworker@communityds.org.au
