what is it?
Healthy Ageing Support Program supports Southern Downs residents over 65 years (and ATSI persons over 50 years) to live at home for as long as possible through activities and models of care that support healthy ageing.
- Assist older people to retain functional capability in their usual activities of daily living, enabling them to live as independently as possible, for as long as possible, in their own homes and communities
- Create meaningful social participation that connects people to family, friends, and community
- Strengthen culturally appropriate health pathways to ensure at-home and primary/community services meet the cultural needs of Aboriginal and Torres Strait Islander peoples
- Provide necessary, appropriate, easy to understand and accurate information in health management, available services, and resource accessibility.
who?
Supporting Southern Downs residents over 65 years of age, or if ATSI 50 years and over.
how does it work?
Priority for referrals are from the Nurse Navigators at the Stanthorpe and Warwick hospitals, and subject to capacity self-referrals and/or GP referrals.
This is a demonstration program funded by the Darling Downs and West Moreton Primary Health Network ending 30th June, 2025 providing funds for a 3 days per week worker.
